Title: Principles and standards for managing healthcare transformation towards personalized preventive, predictive, participative precision medicine

  Global health and social care systems are evolving toward 5P precision medicine—personalized, preventive, predictive, participative, and precise care.
 Effective healthcare transformation requires interoperability and collaboration across diverse domains, disciplines, data models, and ontologies.
 Advanced technologies (AI, micro- and nanotechnologies, edge computing) enable this shift but also introduce ethical, legal, and risk-management challenges.
 Humanistic, moral, and ethical perspectives are essential when managing intelligent and autonomous health systems.
 A formal, ontology-based representation of complex health ecosystems is required to ensure consistency and correctness.
 The ISO 23903 Interoperability and Integration Reference Architecture, aligned with ISO/IEC 21838 Top-Level Ontologies, provides a standardized framework for implementation and interoperability.
